5. Fourteen (14) instruments were in sterile pouches, but did not have an independent indicator strip in pouch

6. Four (4) spray bottles, two (2) with a red liquid and two (2) with a blue liquid, were unlabeled as to the contents

7. Two (2) sharps containers sitting on the counters – not wall mounted

8. Two (2) sets of “sterile” gloves out of the sterile packaging

9. Corrugated cardboard boxes used for storage in the cabinets of procedure rooms

10. Cabinet holding suction machine was rusty

11. Suction machine did not have a date for preventive maintenance (time-based maintenance to ensure reliability)

12. One machine to monitor blood pressure, pulse and pulse oximetry – preventive maintenance was due 3/2013

13. Autoclave spore testing into completed on a routine basis

14. The solution used for ultrasound probe disinfection was not tested prior to each use as per manufacturer’s guidelines.

“The Philadelphia Planned Parenthood failed to ascertain if several minors had been victims of sexual abuse.”

    • Medical Records 1, 2, 3, and 6 stated “13 year old unmarried pregnant patient… no documentation that the facility ascertained if the child had sexual intercourse with an individual who was four or more years older than the child.”

    • Medical Records 4 and 5 stated “MR4…14 year old unmarried pregnant patient… no documentation that the facility ascertained if the child had sexual intercourse with an individual who was four or more years older than the child.”
